Scheduling and Cancellations:
Dr. DeHeer’s flexible schedule accommodates most current and new patient needs. As a courtesy, if you are not able to keep your appointment please cancel to allow the opportunity for another patient to see the doctor. If you miss your appointment and did not cancel, you will be charged a no show fee.
What to bring to your appointment:
Being well-prepared for your appointment will ensure that Dr. DeHeer has all of the needed information to provide the best possible care for you. Please remember to bring the following items to your appointment:
- Identification Card (Driver’s License)
- Insurance Card
- List of any medications you are currently on
- Any relevant past medical records
- If necessary, referral from primary care physician
- New Patient Paperwork (Available below)
When to arrive:
For new patients, we schedule your appointment 10 minutes in advance to allow adequate time for you to complete any necessary paperwork. If you would like to complete the paperwork prior to your visit, please download the forms below. Your initial visit may take up to 60 minutes depending on the nature of your problem.
